Honoring Your Wishes:  Enhancing Person-Centered Care through Advance Care Planning

AMA PRA Category 1 Credits™ (3.50 hours), Non-Physician Attendance (3.50 hours)

1. Identify ways to enhance person-centered care to LGBTQ patients throughout the lifespan.
2. Discuss advance care planning and goals of care conversations utilizing an interdisciplinary approach across the continuum of care.
3. Review IPOST (Iowa Physician Orders for Scope of Treatment) best practices and implementation strategies in primary care, pediatrics, nursing home and hospice settings.
4. Describe lessons learned on providing person-centered and family-oriented care.

Honoring Your Wishes™ is an advance care planning program under the leadership of Iowa City Hospice and a community-wide advisory board.
