Overview
Designed for primary care providers and allied health professionals, this interactive, online CME program explores the challenges of caring for older adults with complex chronic illnesses. Through a multidisciplinary approach, participants will examine the intersection of multiple chronic conditions, functional status, cognitive and psychosocial factors, and patient-centered decision-making.

Target Audience
Specialties - Family Medicine, Internal Medicine

Objectives
  1. Identify key clinical factors—including frailty, functional status, physiologic reserve, cognitive impairment, and psychosocial well-being—that contribute to complexity in the care of older adults.
  2. Apply evidence-informed approaches to prioritize and reconcile competing treatment recommendations in older adults with multimorbidity, frailty, and limited physiologic reserve.
  3. Recognize changes in function, cognition, mental health, and overall health trajectory that may signal increasing vulnerability, decline, or the need to reassess the plan of care.
  4. Integrate patient goals, values, functional status, prognosis, and treatment burden into shared decision-making and longitudinal goals-of-care discussions for medically complex older adults
